Position Summary:
Under the supervision of the Housing and Supports Manager, the Housing Support Worker provides recovery-oriented services at Central Commons– Mental Health Integrated Supportive Housing Program. At Central Commons, the Housing Support Worker assists in carrying out recovery plans with the supportive housing participants. This includes reviewing the recovery plan established by the participant in collaboration with the Housing & Supports Manager and assisting the participant to achieve their goals. The day-to-day tasks involve life skill building (budgeting, cooking, household maintenance etc.) on an individual and/or group basis. This also involves teaching the participant skills to live within an apartment complex with other individuals. The work involves assisting the participant with maintaining formal and informal support networks. The Housing Support Worker will use Psychosocial Rehabilitation and Recovery Principles in carrying out their regular duties.

Qualifications & Experience:
o Post-secondary education in Human Services, Addictions and Community Service, Mental Health Certificate, Social Service Worker or other related disciplines, and/or equivalent experience required.
o Experience in mental health and/or addictions preferred.

Knowledge, Skills and Ability:
o Ability to take initiative and work independently without supervisor on site.
o Knowledge of mental illness and the recovery model.
o Knowledge of addictions and addictions recovery model.
o Knowledge of the principles of trauma informed care.
o Excellent time management, organizational and decision-making skills.

Other Requirements and Conditions of Employment:
• Satisfactory Criminal Record Check (with the Vulnerable Sector Check).
• Satisfactory Child Abuse Registry Check.
• Satisfactory Adult Abuse Registry Check.
• Valid Class 5 Driver’s License.
• Current Vehicle Registration, indicating 3rd Party Liability Insurance of $1,000,000 or more.

Principal Duties and Responsibilities:
Program Participant Recovery Support:
• Participation in Planning and Intervention with Program Participants:
o Reviews Recovery Plan and other documentation to understand participant needs.
o Provides information to the Housing and Supports Manager while supporting an individual’s recovery needs including:
 – Mental health issues/obstacles.
 – Activities of daily living.
 – Recovery & Rehabilitation.
 – Potential Services/Supports required.
• Assists program participants to carry out recovery plans, create structure, and work towards their goals which will support their recovery journey.
• Provide Rehabilitation and Support Services to program participants:
o Intervenes and assists program participants with developing daily living skills.
o Assisting individuals to understand their tenancy responsibilities.
- Acts as liaison between tenant and landlord.
o Assists program participants to acquire skills including:
 – Performing personal hygiene and grooming tasks.
 – Performing household activities including organization, cleaning, laundry, meal planning and grocery shopping.
 – Teaching money-management skills and assist individuals to access financial services.
 – Providing side-by-side support and coaching to assist individuals to socialize.
 – Structuring time and increasing social experience.
o Provides assistance under the supervision and direction of the Housing Manager regarding advocacy, referral, coordination and linking program participants to:
 – Legal and advocacy services.
 – Language and Immigration services.
 – Financial Services.
 – Transportation services.
 – Appropriate services such as home care, mental health, addiction services, financial and educational services to facilitate client access to supports and services.
• Educate and Support Program Participants:
o Coordinate and facilitate services with program participants on a group basis to:
 – Promote personal growth through mental health sessions and activities.
 – Facilitate group activities and events and engage program participants.

Housing Procurement Assistance:
• Participate in development and implementation of housing options as directed by the Housing and Supports Manager:
o Link with inter-sectoral partners (service providers, landlords, housing developers, community groups) to secure housing when client is transitioning from program and on an outreach basis to those on waitlist as caseload allows and as directed by the Housing and Supports Manager.
o Assist program participants one-on-one to “choose”, “get” and “keep” housing.

Secondary Duties:
• Maintains documentation in the electronic record following our documentation guidelines.
• Other duties as assigned by the Housing and Supports Manager and/or Program Director.
